Belt Promotion Registration- Master Method Academy 2021

Belt Promotions will be held in person at the Gymnasium, Inter Denominational Church (19201 Woodfield Road, Gaithersburg, MD 20879) and will consist of performing required techniques for each belt level, as well as new belt presentations.

Covid procedures will be followed, including masks required for all participants, spectators, and instructors. 

Make-up belt promotions will take place on Zoom, and the date/times will be announced following the in-person promotions.

Permission/Waiver

By submitting this form electronically, I certify that the information submitted is corect, and I give my permission for the named student (if under 18 years of age) to participate in the scheduled promotion.  I understand and agree that this promotion involves physical acivity, drills and exercises, and I certify that the student participating is physically able to participate and does not have any medical issues that would inhibit participation.  I further understand and agree that Uchudo International, The Master Method Academy, or any staff members will not be held liable for any injuries, damages, etc. that occur in association with this activity, and the student or parent/guardian of minor student understands and assumes risk of injury by participating in this physical activity in person or virtually online.

By submitting this form electronically, I also understand that photographs & video may be taken at this public event, and I grant The Master Method Academy permission to use and publish photographs or video images of the student, or those in which may be include the student, for purposes including but not limited to: promotional materials,

website, social media, printed publications, and advertising use. I further understand that there will be no compensation for this use and that finished products or advertising copy or the printed matter may be published without my prior review. In granting this permission to Master Method Academy and its legal representatives, I am fully and without limitation releasing Master Method Academy from any liability that may arise from the use of the images.
